Are you a senior or a support person for an aging family member or friend? Do you have questions about services and care options?
If so, you now have a one-stop place to go for information and answers. A local group called Continuum of Care recently published Pathways, a directory that contains informative articles, as well as local provider listings. The directory is being distributed through the Lake Chelan Chamber of Commerce, physicians and providers, local hospital, Chelan Senior Center, Senior Meals and Meals on Wheels, the ministry and others who care about seniors.
The group Continuum of Care began in 2004, when Lake Chelan Community Hospital (LCCH), Regency Manor and Heritage Heights joined together to assure continuity of care as their mutual clients moved from one care setting to another. Since then, more local organizations that provide services to the Lake Chelan Valley’s aging population joined in an effort to support wellness and social opportunities.
Getting the care a patient needs as he or she moves from one care setting to another can often be complicated, said Amanda Ballou, Continuum of Care coordinator and Heritage Heights director. Patients and families have many questions and concerns, and it can be hard to find all the information they need.
The group recognized that gap as they evaluated senior care needs in the Lake Chelan valley,” said Ballou. They presented that analysis to the Community Roundtable in 2008 for validation and additional input. As a result of that meeting, Continuum of Care decided to collaborate and create a services directory specifically designed to serve as a guide for local seniors to navigate the system.
The Lake Chelan Chamber of Commerce and the Chelan Rotary contributed funding for the design and layout of this publication, done by local graphic designer Brandee Borror. Actual publishing costs of the first run of 3,000 issues were funded through the sale of advertising.
Further plans include a website to access up-to-date information and a bi-annual publication of the Pathways guide.
Continuum of Care members include LCCH, Regency Manor, Heritage Heights, Columbia Valley Community Health, Lake Chelan Physical Therapy, Senior Meals and Meals on Wheels, Aging & Adult Care, DSHS Residential Care Services, the Lake Chelan Clinic, Starr Retirement Home, in-home caregivers Bob and Cathy Eire and others who provide services that improve the quality of life for an aging population.
